Home is where the heart is and that is where we are focusing new health and care services for people who are currently unable to leave their home without support.

Home visiting services are being established across Derby and Derbyshire with teams of different professionals providing the appropriate care.

These services ensure that people are better supported (in residential care or their own homes) and help avoid unnecessary hospital admissions.

Home visiting services are being run in the city and county by primary care networks (PCNs) as part of Team Up Derbyshire.

Team Up is creating one team across health and social care in Derby and Derbyshire who see all the people in a neighbourhood who are currently unable to leave home without support. This involves a teaming up of existing services and the creation of additional capacity – with general practice, community care, mental health care, adult social care and the voluntary and community sector all working together and with their local communities.

We now have opportunities to join our team across Derby and Derbyshire, amazing roles with flexible working in great locations, where you can provide the dedicated care our residents need.

    PCNs are currently recruiting for three key roles within their home visiting services:

    • Community GPs
    • Advanced clinical practitioners (ACPs)
    • Enhanced clinical practitioners (ECPs)

    Many people who join Team Up talk about the benefits of being able to spend more time with their patient – rather than the constant pressure of general practice. Team Up team members are encouraged to take a considered, holistic view of the person needing their help, putting in the appropriate support as required. Team Up encourages team members to take a person-centred, strengths-based approach. It’s better for the people requiring their care – and it’s more rewarding for professionals.

Working as a community GP in Team Up

Our survey of GP practices heard that… 

83% of respondents said the Team Up home visiting service freed up GP capacity.

“The service is invaluable to assist with the unmanageable demands… There’s no way we could revert back to managing our own home visits.”

83% of respondents said the Team Up home visiting service enabled housebound patients to be seen earlier in the day.

“Team Up have provided follow-up for patients where needed to help with consistency of care and reduce hospital admissions.”

67% of respondents said the Team Up home visiting service had provided more time to care for patients.

“The home visiting service has made a huge difference in allowing clinical staff to have more time to see and speak to more patients in the practice.”
